Those recommendations become a twelve-week plan with a few small actions a week — add fibre to a meal you already eat, one portion of oily fish — with targets you tick off in the app rather than a document you file away.
After six months you repeat the same assessment and see it beside the first, so the effect of what you changed is measured rather than assumed.
